Insurance & Payment
-
At this time, Elevate GYN & Women’s Health accepts traditional Medicare (straight Medicare) while remaining out-of-network with most commercial insurance plans.
The practice also offers transparent self-pay pricing designed to provide flexibility, accessibility, and clarity for patients.
Even if the practice is out-of-network with your insurance plan, patients may still be able to use insurance benefits for:
Laboratory testing
Imaging studies
Prescription medications
Some specialty referrals
Patients may also use:
HSA/FSA cards
Out-of-network benefits
Superbills for possible reimbursement submission
As the practice grows, additional insurance participation options may be considered in the future. Coverage and reimbursement vary by plan, so patients are encouraged to verify benefits directly with their insurance carrier.
-
“Out-of-network” means that Elevate GYN & Women’s Health does not currently have a direct contract with most commercial insurance companies.
Patients pay the practice directly for services at the time of their visit, rather than the office billing your insurance company for covered care. However, depending on your insurance plan, you may still have out-of-network benefits that allow you to receive partial reimbursement from your insurer.
Many patients are also still able to use insurance for:
Laboratory testing
Imaging studies
Prescription medications
Certain referrals or outside services
The practice can provide a superbill (an itemized receipt with medical billing codes) that patients may submit to their insurance company for possible reimbursement.
Because every insurance plan is different, reimbursement is not guaranteed, and patients are encouraged to contact their insurance carrier directly to understand their out-of-network benefits.
-
In many cases, yes — patients can typically use HSA (Health Savings Account) or FSA (Flexible Spending Account) funds for eligible medical services provided at Elevate GYN & Women’s Health.
This may include services such as:
Office visits
Gynecologic care
Hormone therapy consultations
Certain laboratory tests
Eligible medications or treatments
Coverage and eligibility are determined by your individual HSA/FSA plan and applicable regulations. Patients are encouraged to confirm specific coverage details with their plan administrator if they have questions about eligible expenses.
-
Yes — Elevate GYN & Women’s Health can provide superbills for eligible services upon request.
A superbill is an itemized receipt that includes the information your insurance company may require for possible out-of-network reimbursement. Patients can submit the superbill directly to their insurance carrier if their plan includes out-of-network benefits.
Please note:
Reimbursement is not guaranteed
Coverage varies by insurance plan
Patients are responsible for payment at the time of service
We encourage patients to contact their insurance carrier directly to better understand their out-of-network coverage and reimbursement options.
