“Out-of-network” means that Elevate GYN & Women’s Health does not currently have a direct contract with most commercial insurance companies.

If you choose to use your HSA or FSA card, you are welcome to do so at the time of checkout. Please note that all patients are still required to maintain a valid credit or debit card on file in accordance with our Financial Policy. An HSA or FSA card may be used for payment but does not replace the card on file requirement.

Yes — Elevate GYN & Women’s Health can provide superbills for eligible services upon request.

We kindly request at least 24 hours notice for cancellations or rescheduling. Late cancellations and missed appointments may be subject to a $50 fee in order to protect appointment availability for other patients.

Yes. A valid credit or debit card is required to be on file. The card will be stored securely through our payment processor and will not be charged without authorization.

Patients are welcome to pay using another accepted payment method, including an HSA or FSA card, at the time of service. If an alternate payment method is not used, the card on file may be charged for amounts due, including office visits, procedures, outstanding balances, or applicable cancellation or no-show fees in accordance with our signed Financial Policy.

Your first visit is designed to be comprehensive, personalized, and unrushed. We will review your medical history, symptoms, goals, and any prior testing or treatments. Depending on the visit type, recommendations may include laboratory evaluation, imaging, lifestyle guidance, medication management, or follow-up care planning.

Extended evening and weekend appointments may be available for an additional fee.
